We thank you for making a mail-in donation, because a certain percentage of online donations by credit card is lost to processing fees.

To make a mail-in donation:

mail a personal check and this form to the Anchor House Foundation
hand a personal check and this form to the participating individual/team you are supporting
use your bank's online bill pay feature to have them send us a check on your behalf AND mail or email this form to the Anchor House Foundation
